Privacy Policy

Last updated September 29, 2026

Draft. This document is a placeholder pending legal review and is not yet in effect.

This Privacy Policy explains how Direct Axess Health LLC (“Direct Axess,” “we,” “us”) collects, uses and protects information through the website at directaxesshealth.com and the Direct Axess referral platform.

1. Patient information

Protected health information that providers and practices exchange through the platform is handled as a business associate under HIPAA and governed by our Business Associate Agreement — not by this policy. We use it only to provide the Service to the covered entities we work for.

2. Information we collect about account holders

  • Account details: name, work email, phone, organization, role and National Provider Identifier.
  • Billing details for paid plans, processed by our payment provider. We do not store full card numbers.
  • Security and audit records: sign-in events and actions taken in the platform.

3. This website

This marketing website does not use advertising or tracking cookies and has no forms. Our hosting provider keeps standard server logs (such as IP address and pages requested) for security and operations. [Confirm against the final hosting and analytics configuration.]

4. How we use information

  • To provide, secure and support the Service.
  • To verify provider identity and approve practices.
  • To send service notifications — which never contain patient information.
  • To bill for paid plans and meet legal obligations.

We do not sell personal information.

5. Sharing

We share information with service providers that help us run the Service (for example cloud hosting, email delivery and payment processing), under contracts that protect it, and where required by law. [List of subprocessors — to be completed.]

6. Security

Every account uses two-step sign-in, each organization’s data is isolated, attachments are scanned before they can be opened, and access to patient information is recorded. See Security.

7. Retention

[Retention periods — to be completed by counsel.]

8. Your choices

Account holders can review and update their details in the platform. [State-specific rights and how to exercise them — to be completed by counsel.]

9. Changes and contact

We will post updates here and change the date above. [Contact details to be published at launch.]
